Short-Term Missions Application

If you are interested in being a part of a short-term missions team, you may either obtain an application at the Welcome Center or call the Missions Department at 704-364-5003, ext. 6865 and request that an application be mailed to you.

You may also print an application by clicking on the link below and returning it to the Missions Department in person or mailing it to:

Please note that all applications MUST be completed in full, including a picture and your written testimony. Any application that is submitted without being fully completed will be returned to the applicant.
